(Taken from the "What We Do" section on their website):
We continue to evolve Hamakua Health Center and Kohala Family Health Center according to the model of the Patient Centered Medical Home (PCMH) which puts patients at the center of the healthcare system, and provides primary care that is “accessible, affordable, continuous, comprehensive, family centered, coordinated, compassionate, and culturally effective.” Services for the low-income, uninsured are provided on a Sliding Fee Scale that is based on family income and size.
As the health care home for nearly 8,000 residents of North Hawaii, Hamakua Health Center and the Kohala Family Health Center provide a comprehensive range of health, educational and preventive social services while treating the whole person in a quality, cost efficient and caring environment. We not only offer primary medical care services, but also supportive services that include behavioral health services, oral health services, patient and family education, family planning, women’s health services, support groups, translation services and transportation to and from appointments. We believe that each person deserves access to outstanding, state-of-the-art health care and highly skilled health care providers, and we are able to do this through generous federal, state, and county grants; donations from private donors; and the dedication of all our employees. Quite simply: We believe in caring for ohana… and caring for you.
