I recently had sex with my girlfriend. (I'll try not to be too graphic) After about 10 minutes I pulled out and noticed the condom had slid up about and inch from the base of my penis. I took it off and noticed pre-cum inside the tip of the condom. The top of my penis that was still covered by the condom was a little wet as well. I wiped it off with a towel and put a new condom just to be safe. Could this be considered a condom failure?

Condoms can slide around a little bit and often will, that's fine. What you are describing is not a condom failure.

If the condom didn't fall off completely or didn't tear then it's not a condom failure.
