I am on Alesse 21 (and have been for about a year, and before that Yasmin 21 for a few more years), and I'm just wondering what kind of time frame I can take my pill in. I always, always always take it between 10:00 PM and 12:00 AM and was just wondering if that's OK for being protected. There's lots of vague stuff about "approximately the same time each day" and "within a few hours," and I'd just love to clarify how accurate I have to be. My boyfriend and I aren't having sex yet, but there's definitely a possibility of pre-cum or a tiny bit of semen coming in contact with down there when we're fooling around, and I'd just like to be as safe as possible, as well as not having to ruin the moment by being practically clinically sterile about wiping our hands before sticking them down there, although there's never, like, buckets of fluid being transferred. With a combined pill (two kinds of hormones) like Alesse 21, you have about three hours either side of your usual pill taking time in which to take it. So taking it at midnight when you usually take it at 10 pm is just fine.

Per the risk from manual sex, that isn't about pregnancy so much as it is about infection, so even if you don't need to get any pre-ejaculate or ejaculate off your hands, having clean hands before they come in contact with your genitals is a good idea from an infection standpoint.

That's a relief. We do make sure to be clean, so that probably won't be an issue. I do have some anxiety issues that I'm working through around pregnancy, so that part of the question wasn't so much about the actual risk as it was to settle my mind. [Smile] So thanks for setting me at ease, as well as giving me some good info about when to take my pills.

I also have another question. My nipples are sort of sore right now, and the left one more so than the right. Is that a result of birth control? I'm due for my withdrawal bleed in the next few days, so if it doesn't go away by the time that's finished, should I see a doctor?

Sore nipples can definitely be a side effect of birth control, but sometimes it just happens for no obvious reason. Breast or nipple soreness isn't necessarily connected to your withdrawal bleed either, so it may go away after that, it may not. If it keeps on for several days, it wouldn't be a bad idea to check in with your doctor.
