Its my second month of taking the pill and I missed a pill the second day of my last active pack. I was sick and vomited. Would my pill be completely effective the first day I start my next active pack?

Okay, so first things first: when you miss a pill, you always need to make it up like your patient insert in every pill pack will explain. (How irritating your doctor didn't mention this: it's about the most common thing to know about oral contraceptives, and not new information at all.)
Since you didn't, you have to figure that whole last cycle was potentially compromised, and you want to wait to go without your backup method until you're at least 7 days into this new pack.

Missing one pill doesn't impact the next cycle IF you make up that missed pill. Since you didn't make it up, the whole of your last cycle may have been compromised, and you need to figure that you're basically starting the pill all over again from scratch, hence using a backup method for at least the first 7 days of this new pack.
Make sense?

Oh I see, ok. If I vomited nearly 3 hours after taking the pill, should I consider it totally missed?

If it was a whole three hours later, I wouldn't say it was missed at all. Digestion usually happens within the hour, not hours after ingesting something.

I took it around 5:40 and then vomited around 9:15. I wasnt sure if I should have consider it missed

Can I consider my pill effective this cycle then? I didnt make up the pill since my doctor didnt mention to but considering I may not have even really missed the pill.

You can consider your pill has been effective this cycle, as you would have already digested your pill by the time you vomited. The pill would have already been absorbed by your body, so wouldn't have been expelled from the body when you vomited.
Therefore this doesn't count as a missed pill and you are still as protected as yould would have been had you not vomited.

When does it become a problem? My doctor only said vomiting and diarrhea could become an issue. I was reading other posts but thought it better to just ask.
An hour after taking the pill....is that when I should make up the pill after vomiting or having diarrhea?
