No sexual act is guaranteed to produce pleasure for every person all the time. It depends on what that person enjoys most, how they're feeling at the time, what they want, etc.
If a guy ejaculates, then yes, that usually means he's had an orgasm.
But you can experience pleasure or have a good time without having an orgasm. And someone could have an orgasm but still feel emotionally uncomfortable or unhappy about the sexual activity.
And generally, if you want to know if someone's enjoying themselves, it's a very good idea to ask them.
