Would a diaphram with no spermicide used as back up method to condoms offer so little in terms of protection as to not be worthwhile?

It's tough to say.

Because of concerns about spermicides, there was a lot of DIY thought that went into using cervical barriers without them, or with homemade sperimicdes, in the past when they were more used, especially in the 70s.

The thing is that all the broad studies done on cervical barriers include use of a spermicide, and that's a pretty big part of their design: in other words, the spermicide is part of the design of the device, really.

All that given, I think the best we can say is that we don't know for sure, but we can be sure they would be substantially less effective without some form of reliable spermicide. If that's the only option someone has as a backup, and spermicide is off the table, I'd feel comfortable it likely provides some extra protection, so is likely better than no backup, but I just couldn't say how much better.
