Why do condoms fail if they are used correctly and consistently?

Sometimes, no matter how diligent someone is at using a condom correctly, it will still break. Very little, if anything, in life is guaranteed to work 100% of the time.

If a condom breaks, though, it's something that the person will know about right away. So, if someone uses a condom for all genital contact, every time they have genital contact, and the condom doesn't break, they can know that getting pregnant is as unlikely as--oh, I don't know--as finding a thousand bucks in cash just lying on the sidewalk.

But I heard that sometimes, sperm could leak from the base of condoms, does it count as perfect use failure too?

Condoms are designed so that they don't leak. If a condom is left on after ejaculation then it might leak, but that isn't correct use as it's supposed to be removed.
