My inner labia is kind large - kinda like a tongue sticking out of a mouth - and on the side where the inner labia meets the outer labia, it hurts/stings as if there is a cut. Right next to that area is a hard white lump (sometimes a couple). Every so often that area itches pretty bad. I've had the bumps on and off since late 2008. (I know I know, why have I waited so long to ask about it.) Sometimes when whatever it is flares up, it burns/stings when I wipe with TP or scrub it with a cloth in the shower. It hurts the worst after I have had my period, in which I use only cloth diapers and occasionally an overnight always pad. I never use tampons. I think it hurts more after my period, because the pads dry out/irritate my skin.
I have been tested for STDs and had a good pelvic exam, I'm all clear. The doctor mentioned something about treating it with warm water and that it wasn't a big deal. I didn't really catch everything, because this particular doctor was being really rough with me and I was going through the most painful pelvic exam of my life.
What could this bump be, and how do I treat it?

I'm flummoxed that you had a doctor look at these who made clear they weren't an infection, but also didn't tell you what they were. At the same time, it sounds like that doctor did a poor job with you, period.
Especially since, from the sounds of things, and your being clear with it being anything else, it sounds like you're dealing with benign cysts, here.
Have you tried using warm/hot compresses on them to see if that helped?

I've never really tried to treat them. The painful/itching/bumpy situation only lasts a few days a month. Can cysts come and go like that? I've never had them prior to 2008.

It's actually very common for vulval/labial cysts to kind of flare up and flare down sometimes for various reasons, including changes in body chemistry, temperature, friction, etc.
I am a bit concerned, though, they they feel painful or itchy. Some soreness can happen when and if they get bigger, but I'd say itchiness isn't that common.
I know you had a bad experience with this last GYN, but might you be willing and able to seek out a second opinion, ideally from someone who provides better care?
If not, am I getting it right that this seems related to your period and maybe your pads? have you tried changing how you manage your flow -- like maybe by only using washable pads, or switching to 100% cotton, no-additive pads -- to see if that makes any difference? i ask that because with the itching, I wonder if this isn't about an allergy or sensitivity.
