Using a hormonal birth control method together with condoms is really as close to zero as pregnancy risks get. If you've used both consistently, the chance of pregnancy is really null.
Sometimes, a woman will skip a withdrawal bleed while on the pill for no reason. Most information leaflets will say that this is nothing to be concerned about as long as all the pills in the pack have been taken correctly.
