My girlfriend was on ortho tri cyclen lo for 4 years. She was off for 2-3 weeks because of insurance reasons. She has been back on for about 2 weeks. The pack says its effective after 7 days. We had sex twice tonight using condoms. AFter the first time was over, I went to the bathroom took the condom off disposed of it, then washed my hands with warm water and anti bacterial soap. Then dried my hands and put on a second condom and went to town again, I did not finish in her. When I came out of the bathroom she revealed she took her pill 13 hours late today. Would her pill be effective at all? After washing my hands would that kill any sperm left on my hands before I put the new condom on?

The effect of a birth control pill comes from all the pills taken in the 7 days before so even if she was late taking one during the day there was still probably enough of the contents in her system over time to be effective. We'd usually recommend condom use for a set time afterwards to be on the safe side but you already use condoms.
Condoms are a super effective option on their own. And not only do I not think sperm would be able to be on the outside of a new condom without being obviously visible, but given all your hand washing it seems even more unlikely.
I hope that's helpful.
