ok i know that when the guys cums, you are supposed to grab onto the condom so no sperm gets out. but can any sneak out the top while you are having sex? or just after he cums?

If you are using a condom properly, there should be no need to grab the rim of the condom upon ejaculation. It's when you withdraw that you should be holding on so the condom doesn't slip off.
If you have a snug fitting condom and left space at the end for semen to accumulate, there is no reason that anything should leak.

It's really a good idea to use lube - the friction of sex can be pretty intense sometimes, and lube helps keep condoms from breaking. Not using lube makes condom failure more likely.
