does it matter if you take the placebos, or whatever they are called on time. The ones that are meant to be taken during your period. also if you have to take a topical anti-biotic, does the screw with the birthcontrol?

Nope - the placebos don't have an effect, they're just there to help you keep taking the pills on schedule.
As far as I know, topical antibiotics shouldn't affect the effectiveness of birth control pills, but I'd recommend checking with the doctor who prescribed the antibiotics to be sure.
