There is no universal way for asking a girl out. If there's someone you'd like to get to know better, you can simply suggest hanging out one-on-one for a low-key activity, such as having coffee.
There is no need to tell someone up-front about your dating history, but that's a topic that often comes up naturally sooner or later, and when it does, you'll simply want to be honest.
