Say if you were to have unprotected sex. The male whom you had sex with has a sexual transmitted disease. Is it transmitted at the time it is inserted or when the male ejaculate?

If you're having unprotected sex with someone, there doesn't even need to be insertion for a sexually transmitted disease to be passed. It does depend on the disease, but since many diseases do not show external signs, you can never be sure without a test, and you simply cannot be safe without a condom.
