is a pill (alesse, in particular) considered "late" after 45 minutes - 1 hr? is it still at full effectiveness?

Nope, you're perfectly fine as long as you've been on the pill for awhile. The only cause for concern is when you're 3 hours or more off from your regular time, and even then the effectiveness is barely affected, if at all.
