This thing sometimes happens to my ears when I have an orgasm. It was common when I was a young teenager and now only happens two or three times a year.

What happens is I sort of go deaf for a few minutes. I can barely hear anything and it feels like I'm underwater. It's a stronger version of what sometimes happens on planes or when driving and the altitude changes--it must be a pressure thing? It's slightly painful but not terrible. It goes away after about ten minutes. Should I be worried about my ears?

(Just because I'm curious too)

I tend to have that happen if the orgasm happened to be particularly intense.

Sounds are a bit muted, and sometimes I can hear a slight ringing. It ususally goes away in like 5 minutes.

I've always wondered about it, but since my hearing seems to be fine otherwise, I've gotten used to it.

It sounds like a pressure change. You know how when you plug your nose and try to blow, your ears will plug up? Probably you're tensing certain muscles in your head/face when you orgasm, and the pressure is building up in your ear canal. After a few minutes, it is released naturally. If it's causing you problems, you should see a doctor. Otherwise, maybe bring it up at your next physical/STI check in case your doctor has any thoughts.
