Is there any way that the condom could have broke or torn without either of us have knowing?

I think it's pretty easy to tell, because after the man pulls out, his ejaculate will be leaking out of the condom. Ejaculate may also leak out of the woman. (This has only happened to me once, though, so I'm not an expert.)

It's possible that there could have been small tears or holes in the latex, especially if the condom was old.

If you're worried about pregnancy, please take a test!
